updating their medical history
reviewing and signing treatment plans and consent forms
updating contact information
paying deposits and outstanding balances
The new addition of the FP17 PR forms to the Patient Portal completes the online, contactless experience.

How does it work?
It's so easy, you don't have to do anything, Dentally's Patient Portal sets it up for you automatically as long as certain criteria are identified.
Exclusively available online via the Patient Portal, patients can access, read and sign the digital NHS PR form on whatever device they prefer - mobile, tablet or computer.
NHS patients using the electronic NHS patient declaration form, reduce the need for paper admin when the patient arrives at the practice at their allocated appointment time and encourage social distancing with your reception team.
The form is simple to complete and sign, and if a person needs to sign on behalf of a patient - for example as a parent - then they can easily do that too. Once the form has been submitted, it will appear in the patient’s correspondence tab in Dentally.
